The RSI loop wedges on old blocked initiatives filed before the mandatory-criteria gate (INI-2199). Elrond Author follow-on path copies parent criteria verbatim; when the parent has ZERO criteria (e.g. INI-999996) it files with an empty list, the gate refuses, and the loop retries the same retrigger every cycle with no backoff (observed 00:37 -> 03:56, dozens of wasted cycles + lock contention).

Scope: 1. Author follow-on path: when both the parent and the LLM yield zero criteria, explicitly ask the Author LLM to draft 1-3 measurable USER-VISIBLE outcome criteria from the parent description + retry intent (outcome-phrased to satisfy the INI-2205 process-only check), and record a decision on the new follow-on noting criteria were drafted at retrigger because the parent predates the gate. 2. RSI loop guard: after 3 consecutive identical filing/retrigger failures for the same family, tag the original (rsi-retrigger-stuck) with the error, post a decision for the operator, and SKIP that family until the tag is cleared — the loop must never burn every cycle on one unfileable family.

Work the blocks for your work-type: template -> design -> code -> test -> review. Do the real work; record decisions on the initiative as you make them. 4. Reality is recorded for you at close -- code (files changed), each decision, and the twin-review verdict become real activities tied to this initiative and show in the Activity Tracker like a planner run (CAROL-INI-1840). No dummy rows. 5. Keep the initiative status moving; it parks in 'reviewing' and is tagged uat-pending for you at close (CAROL-INI-1836), so the stuck-watchdog leaves it alone until UAT. 6. Close runs the gates (design/architecture compliance + caller-audit). If a gate flags something pre-existing or unrelated to your change, waive it with a clear written rationale -- audit, don't skip. 7. Bypass skips the planner's auto-orchestration, NOT the standards.
